We're so glad you're interested in joining the Verizon West Chapter. To become a member we ask that you complete  The Membership Application.  By clicking on this link, you may print out the application. After completing the form, mail it along with your check to our address listed on the top of the form.

If you are a current Verizon employee, we would like you to also fill out the Payroll Deduction Form. due to the fact that annual bills are no longer sent out. This deduction will not take effect until  Pioneer year July 1, 2003 to December 31, 2004.  Annual dues are $15, which will be collected once a year by payroll deduction the first pay period in the month of July beginning in 2003.
